Practice Policies

PAYMENT

You are responsible for bringing accurate payment at the start of every session. All services may be paid with cash, check, credit card, Health Savings Account or Flexible Spending Account cards or insurance (see below) at the time of service.

Should a personal check bounce back to us, any accrued banking fees will become your responsibility. NourishRX, LLC requires that you provide a credit card information to be held on file. A signed authorization form will be provided at the start of the initial session. This agreement is valid for one year. If the credit card on file declines, the balance will automatically be pushed over to a collections agency after 60 days from the scheduled date of service. If insurance denies coverage, you will be notified with a pending charge notification. If you have a credit card on file, this card will be charged 5 days after the notice is sent. If there is no credit card on file, you will have an obligation to pay the balance in full 60 days from the scheduled date of service.

INSURANCE

At this time, we are credentialed with:

  • Blue Cross Blue Shield

  • Harvard Pilgrim

  • Tufts Commercial Health Plan

  • United Health Care

  • AllWays Health Partners.

Your specific plan, even if it is with one of the above insurance companies, may or may not cover nutrition counseling. It is your responsibility to understand your health insurance benefits. If your insurance does not cover any or all of our appointment (such as in the case of unmet deductibles, uncovered services, inaccurate copays, denied claims, etc.), we will send you an invoice for the balance. It is your responsibility to pay this upon receipt. If your health plan changes, you must notify NourishRX, LLC prior to your next appointment. Failure to do so may result in rejected claims, which will then be your responsible for paying.

If we do not take your insurance, we will provide you with a superbill (fully coded receipt) to submit to your insurance as they may reimburse you directly for all or some of what you paid. Please note that a Superbill does not guarantee reimbursement.

CANCELLATION POLICY

Keeping your regularly scheduled visits will allow for the best outcomes in your nutrition treatment. We request that all appointments you cannot make are canceled at least 24 hours in advance of your appointment to avoid a $75 cancellation fee. This fee cannot be billed to insurance.

We understand that life happens, so should you need to cancel your visit within 24 hours the first time will be waived. Subsequent late cancels will be charged at the rate of $75 regardless of appointment type or length of visit.

If you need to cancel your appointment, please call us at 978- 927-0990 or send an email to info@nourishrx.com. We cannot accept cancellations or messages via text or directly to your dietitian’s email address.

Frequent rescheduling (defined as 3 rescheduled appointments over the course of 2 months) and/or 2 late cancellations over the course of 2 months, will result in the forfeit of all future appointment times. You will need to call the office to reschedule and reinstate future appointments.
