PRACTICE STANCE AND POLICIES
It is NourishRX's responsibility to create an environment where clients feel safe. Clearly defined professional boundaries benefit both the RD and the client by ensuring that behaviors, words and actions within the RD-client relationship are always focused on what’s best for the client. NourishRX RDs have a responsibility to act in the best interest of clients at all times.

PAYMENT
You are responsible for bringing accurate payment at the start of every session. All services may be paid with credit card, Health Savings Account or Flexible Spending Account cards or insurance (see below) at the time of service.

NourishRX, LLC requires that you provide credit card information to be held on file. A signed authorization form will be provided at the start of the initial session. This card will be used for copays, deductibles, denied claims and late fees unless otherwise noted by the client. The credit card on file may be updated at any time in the NourishRX Portal. If there is no credit card on file, you will have an obligation to pay the balance in full 60 days from the scheduled date of service.

INSURANCE
At this time, we are credentialed with:
Aetna
Blue Cross Blue Shield
Harvard Pilgrim
United Health Care
Mass General Brigham Health Partners (formerly AllWays Health Partners)

Your specific plan, even if it is with one of the above insurance companies, may or may not cover nutrition counseling. It is your responsibility to understand your health insurance benefits prior to the start of treatment with NourishRX, LLC. If your insurance does not cover any or all of our appointments (such as in the case of unmet deductibles, uncovered services, inaccurate copays, denied claims, etc.), we will send you an invoice for the balance. It is your responsibility to pay this upon receipt.

If there are any changes made to your health insurance during the duration of your treatment at NourishRX, LLC, you must notify NourishRX, LLC prior to your next appointment. Failure to do so may result in rejected claims, which will then be your responsibility for paying.

If we do not take your insurance, we will, upon request, provide you with a superbill (fully coded receipt) to submit to your insurance as they may reimburse you directly for all or some of what you paid. Payment must be made, in full, to NourishRX, LLC before a Superbill can be provided. Please note that a Superbill does not guarantee reimbursement.

APPOINTMENT & CANCELLATION POLICY
Keeping your regularly scheduled visits will allow for the best outcomes in your nutrition care. We request that all appointments you cannot make be canceled at 1 business day * (24 hours) in advance of your appointment to avoid being charged a full session fee of $200. This is necessary because a time commitment is made to you and is held exclusively for you. This cancellation fee cannot be billed to insurance. 
*Business day is defined as Monday-Friday. Please note, this means that if you have a Monday appointment, you will be responsible for canceling this session on Friday to avoid a session fee. We will be sending out a notice of appointment 80 hours prior to your scheduled session to ensure you have ample time to cancel your standing appointment if necessary. 

Late cancellations (defined as cancellations less than 1 business day [24 hours] prior to your appointment start time) of 1:1 medical nutrition counseling will be charged at a rate of $200 regardless of appointment type or appointment length. 
If you are able to reschedule your appointment to the same week of your previously scheduled session, there will be no fee. If you are unable to reschedule your appointment in the same week, there will be a $200 charge. 

If you need to cancel your appointment
within 24 hours of your appointment, please call us at 978- 927-0990 or send an email to info@nourishrx.com with your dietitian cc’ed. We cannot accept cancellations via text, messages through the client portal or directly to only your dietitian's email address. 

Appointments are expected to start promptly at the booking time. If you are running late to a scheduled appointment, please call or email the practice so that the dietitian can be notified. The dietitians will wait 15 minutes from the scheduled start time for 60 minute sessions and 10 minutes for any appointments scheduled for less than 60 minutes before canceling your appointment. Dietitians will both call and email you with messages to connect prior to canceling the appointment. If you have not shown up past the agreed upon time (see above) the appointment will be canceled and charged $200 unless appropriate communication is made to the practice.

If there are any technological delays from the portal access or issues with internet connectivity, the practice will provide you with an alternative link to proceed with for that particular session. If an emergency, illness, or other last-minute change occurs on our part, the practice will provide you with notice as soon as possible along with complimentary rescheduling and no cancellation fee.

Frequent rescheduling (defined as 3 rescheduled appointments over the course of 2 months, and/or 2 late cancellations over the course of 2 months) will result in the forfeit of all future scheduled appointment times. You will need to call the office to reschedule and reinstate future appointments.

Appointments are booked on a first-come, first-served basis. It is your responsibility to ensure that you are keeping up to date with scheduling visits per your dietitian's recommendation to avoid being unable to book at your desired appointment time. 

NourishRX can not hold treatment times with specific providers should there be any leave from our consistent care
. We cannot guarantee that your provider will have availability following your reengagement with the practice.


SOCIAL MEDIA 
All RD's are restricted from friending, linking or interacting with clients on personal social media accounts as this could potentially interfere with the professional RD-Client relationship. It is up to the client to determine whether to connect with the NOURISHRX business social accounts and NOURISHRX will not solicit any social media interaction with individual clients. As always, no nutrition advice or medical details will be discussed through these platforms.

GIFTS
Employees of NOURISHRX are unable to accept gifts of any kind from clients, former clients or family members of clients. This includes cash and gift cards.Written cards may be accepted as long as they do not contain cash or gift cards.

SERVICES
Medical Nutritional Therapy services (“MNT”) are nutritional treatments which include personalized nutritional assessments in combination with the treatment of any diagnoses made  therefrom through therapeutic and counseling services. NourishRX’s MNT services are available only to individuals in the Commonwealth of Massachusetts, Arizona, California, Colorado, Michigan, New Hampshire, New Jersey, Nevada, Maryland, Virginia. NourishRX does not offer its MNT services to individuals outside of the states listed. MNT Services are covered by a variety of health insurance plans, but coverage is dependent upon your specific plan. Services which do not qualify as MNT are not billable to insurance. 
NourishRX’s services which do qualify as MNT include, but are not limited to:
1:1 Nutritional Counseling to treat a diagnosis provided by a medical doctor.
 
NourishRX’s services which do not qualify as MNT include, but are not limited to:
Support Groups, online communities, courses, toolkits, exercise and movement, meal coaching and consulting
