PAYMENT
You are responsible for bringing accurate payment at the start of every session. All services may be paid with cash, check, credit card, Health Savings Account or Flexible Spending Account cards or insurance (see below) at the time of service.
Should a personal check bounce back to us, any accrued banking fees will become your responsibility.

NourishRX, LLC requires that you provide credit card information to be held on file. A signed authorization form will be provided prior to the start of the initial session. This agreement is valid for one year. If the credit card on file declines, the balance will automatically be pushed over to a collections agency after 60 days from the scheduled date of service. If insurance denies coverage, you will be notified with a pending charge notification via email. If you have a credit card on file, this card will be charged 5 days after the notice is sent. If there is no credit card on file, you will have an obligation to pay the balance in full 60 days from the scheduled date of service.

INSURANCE
At this time, we are credentialed with:
Blue Cross Blue Shield
Harvard Pilgrim
Tufts Commercial Health Plan
United Health Care
AllWays Health Partners

Your specific plan, even if it is with one of the above insurance companies, may or may not cover nutrition counseling. It is your responsibility to understand your health insurance benefits prior to the start of treatment with NourishRX, LLC. If your insurance does not cover any or all of our appointments (such as in the case of unmet deductibles, uncovered services, inaccurate copays, denied claims, etc.), we will send you an invoice for the balance. It is your responsibility to pay this upon receipt.
If there are any changes made to your health insurance during the duration of your treatment at NourishRX, LLC, you must notify NourishRX, LLC prior to your next appointment. Failure to do so may result in rejected claims, which will then be your responsibility for paying.
If we do not take your insurance, we will, upon request, provide you with a superbill (fully coded receipt) to submit to your insurance as they may reimburse you directly for all or some of what you paid. Payment must be made, in full, to NourishRX, LLC before a Superbill can be provided. Please note that a Superbill does not guarantee reimbursement.

CANCELLATION POLICY & APPOINTMENT POLICY
Keeping your regularly scheduled visits will allow for the best outcomes in your nutrition care. We request that all appointments you cannot make are canceled at least 24 hours in advance of your appointment to avoid a $100 cancellation fee. This is necessary because a time commitment is made to you and is held exclusively for you. This cancellation fee cannot be billed to insurance.
NourishRX, LLC respectfully understands that things come up that are out of your control, so should you need to cancel your visit within 24 hours the first time will be waived. Subsequent late cancels will be charged at the rate of $100 per session regardless of appointment type or length of visit.

If you need to cancel your appointment within 24 hours of your appointment , please call us at 978- 927-0990, send an email to info@nourishrx.com, otherwise please cancel through your electronic email/text confirmation. We cannot accept cancellations or messages via text or directly to your dietitian's email address.

Appointments are expected to start promptly at the booking time. If you are running late to a scheduled appointment, please call or email the practice so that the dietitian can be notified. The dietitians will wait 15 minutes from the scheduled start time before canceling your appointment. The appointment will be canceled and charged as a no show unless appropriate communication is made to the practice.
Frequent rescheduling (defined as 3 rescheduled appointments over the course of 2 months, and/or 2 late cancellations over the course of 2 months) will result in the forfeit of all future appointment times. You will need to call the office to reschedule and reinstate future appointments.

Appointments are booked on a first come, first served basis. It is your responsibility to ensure that you are keeping up to date with scheduling visits per your dietitians recommendation to avoid being unable to book at your desired appointment time.

SERVICES
Medical Nutritional Therapy services (“MNT”) are nutritional treatments which include personalized nutritional assessments in combination with the treatment of any diagnoses made therefrom through therapeutic and counseling services. NourishRX’s MNT services are available only to individuals in the Commonwealth of Massachusetts. NourishRX does not offer its MNT services to individuals outside of the Commonwealth of Massachusetts. MNT Services are covered by a variety of health insurance plans, but coverage is dependent upon your specific plan. Services which do not qualify as MNT are not billable to insurance. 

NourishRX’s services which do qualify as MNT include, but are not limited to:
1:1 Nutritional Counseling to treat a diagnosis provided by a medical doctor.

Navigating Recovery Support Group
Virtual Dietary Education Program
Treatment Care Option Guidance
Parent & Caregiver Support Community
